MAHA's 2023 Conflict of Interest form is now online. 

Each board member, staff and volunteer shall be provided with and asked to review a copy of this Policy and to acknowledge in writing that he or she has done so. 

Annually each director, officer, employee and volunteer shall complete a disclosure form identifying any relationships, positions or circumstances in which he or she is involved that he or she believes could contribute to a Conflict of Interest.

You will need to register as a USA Hockey volunteer (no charge) for the 23-24 season before you begin this process as you will be asked for your confirmation number during the registration.

All Directors must also complete or the current season on their Background Check and SafeSport Training. Other volunteers that have routine and regular access to minor participants must also be current on their Background Check and SafeSport training.
