This form is to be used by all Adult Teams to enter the MAHA State Playoffs.   

Men and Women's Divisions will be held March 14-16th at Big Boy Arena.

• All players will receive a welcome gift and a beer or soft drink ticket for each game.
• 3 game guarantee - 3-12 minute periods.
• Champions will receive personalized team banner, trophy, individual medal and gift.  Runner-ups will receive a trophy and individual medal.

Entry fee is $1000.  A deposit of $250 is required at the time of signing up.  Any divisions that are not filled will be refunded.   Final payment will be due on 1/15/2025.

All players must be registered with USA Hockey.
